C/o 

Swelling in the anterior aspect of the neck for 1 month

History of Present Illness

Swelling : Location ,Onset ,How noticed, Initial size, Progression , Final  size . associated with pain fever ,trauma or Sinus  discharge.

Pressure symptoms 

Dyspnoea : Dyspnoea on exertion , Orthopnoea , PND and dyspnoea at rest

Change of voice 

Chocking 

Dysphagia 

Functional symptoms

CNS: Anxiety .Nervousness. Decrease sleep .Tremors. Falling objects

CVS: Palpation ,chest pain and lower limb oedema

Metabolic & GIT :Wt loss despite good appetite ,Preference of cold Weather , Diarrhoea .

MS: proximal myopathy (inability to camb his hair or stand from squatting),skin rash and loss of hair

Gu: Monorrhagea irregular cycle  , Urine amount and Colour , Galactorrhoea 

Symptoms of Malignancy

Symptoms of anemia(Fatigability-SOB-Palpitation). Fever with sweating

Symptoms of Metastesis

GIT:Jaundice. abdominal distension

 CVS +Respiratory:Cough with haemoptysis ,Chest pain lower limb oedema

CNS: loss of consciousness . Headache . Convulsion
